New Provider Orientation- Agency Participant Registration

Registration and Attendance Details:

• All attendees must pre-register before participating.
• If you do not pre-register you will not be invited to participate.
• No credit is given for late arrivals or early departures. Each participants will be required to attend both sessions with a video camera turned on and complete the survey following the training to have a successful completion of the new provider orientation.
• You must complete a Prospective New Provider Agency Orientation Registration Form, see below.
• Space is limited. Optional: up to 2 persons from each prospective provider may register, but is not required.
• If more than one person is registering, you must complete a separate registration form for each person.
• Registrations are on a first-come, first-served basis.
• Participants are required to sign-in and sign-out to receive credit for attending.

The New Provider Orientation Training on Tuesday May 17, 2022 and Thursday May 19, 2022 via WebEX is now available for registering.
