Person Centered Planning Process For Medicaid Waiver Services

Through the Life Choices System Transformation efforts and in accordance with Federal Home-and Community-Based Services Medicaid requirements, the Division of Developmental Disabilities implemented a new person-centered planning process for community-based Medicaid Waiver services July 1, 2017. This webpage shares important documents and information with individuals, providers, independent service coordinators, and other interested stakeholders.

Individuals served in community-based services (e.g., Community Integrated Living Arrangements (CILA), Day Programs, Home-Based Services and Supports (HBS)) are required, under federal regulations, to have plans that are updated at least annually.
