Effective May 1, 2017, Molina Healthcare of Illinois (Molina) will no longer be part of the Medicaid Managed Care Family Health Plan (FHP) Program, the Integrated Care Program (ICP), and the Medicare-Medicaid Alignment Initiative (MMAI) Program in six southern central Illinois counties. The counties affected are Christian, Logan, Macon, Menard, Piatt, and Sangamon.

Molina members residing in six Southern Central Illinois counties (Christian, Logan, Macon, Menard, Piatt, and Sangamon) will be dis-enrolled from Molina Healthcare on April 30, 2017. Beginning May 1, 2017, these persons will be enrolled in the regular Medicaid fee-for-service program.
MMAI Molina members may choose to enroll in a Medicare Advantage plan or Original Medicare with a Part D plan to cover prescriptions. Persons who do not make a Medicare choice by April 30, 2017 will be automatically enrolled in Original Medicare fee-for-service and a SilverScript Insurance Company Medicare Part D prescription drug plan.
Persons who were enrolled in Molina Healthcare may not be able to keep the same medical providers. Persons with questions about the transition process should be referred to the Molina Member Services at 1-855-687-7861 (TTY: Illinois Relay at 7-1-1) through April 30, 2017.
Beginning May 1, 2017, refer persons needing help finding a fee-for-service provider to the Illinois Health Connect helpline at 1-877-912-1999 (TTY: 1-866-565-8577).
Persons enrolled in regular Medicaid fee-for-service should take their HFS medical card to pharmacy and doctor visits to get services. Customers who need a replacement medical card should call:

Persons living in Champaign, DeWitt, Ford, and Vermilion counties, and enrolled with Molina Healthcare in the FHP Program or the ICP Program can remain enrolled with Molina Healthcare or they can choose to dis-enroll and become fee-for-service participants. There are no changes to their healthcare plan. Newly eligible persons can also choose to voluntarily enroll with Molina Healthcare in these counties. Persons residing in these counties must call the Illinois Client Enrollment Broker at 1-877-912-8880 (TTY: 1-866-565-8576) to request to be dis-enrolled or to voluntarily enroll with Molina Healthcare.
