1230-GA-Special Education - Grants for Infants and Families- Early Intervention - Credentialing

To be eligible for a FY 18 contract, all grantees must complete the Pre-Qualification Process, Internal Controls Questionnaire, Programmatic Risk Assessment and Grant application. The Grant Application and Programmatic Risk Assessment are to be completed for every program/grant. See Grant Application Information and Instructions webpage for more detail.

At the time of application, all Bureau grantees must submit for each grant program the following documents:

Applicants are recommended to begin completing the FY18 budgeting process using the fillable PDF version of the Uniform Grant Budget template.  Please note the applicant will only use this as a budgeting tool to prepare a draft budget to enter into the IDHS CSA system.  Important Note:  The final budget document MUST be submitted via the IDHS CSA system.  A PDF or paper copy will not be accepted

The completed documents are to be emailed by 4/17/17 to the Bureau email address Nick.Marsters@illinois.gov with the subject line stating:

Last 4 digits of CSFA #1230

Provider Organization Name - For example, 1230 Jane's DV agency or 1230 HOPE agency

Note: everything is being tracked by the CSFA number, so it is important that all required materials and correspondence contain this information.
