As a part of the requirements instituted under the 2 CFR 200 and Grant Accountability and Transparency Act (GATA), grantees of the Illinois Department of Human Services (IDHS) must answer questions included in the Programmatic Risk Assessment Survey tool provided by the Governor's Office of Management and Budget Grant Accountability and Transparency Unit. The surveys have now been posted to the IDHS website.

Grantees may access their location by going to webpage at https://www.dhs.state.il.us/page.aspx?item=86068 or they may choose the following links from the IDHS homepage: For Providers/Grants/Grants from IDHS/Programmatic Risk Assessment Surveys.

Once you have accessed appropriate location, choose Programmatic Risk Survey - Division of Alcoholism and Substance Abuse. Grantees have 30 days to complete the survey.

The Programmatic Risk Survey must be completed in addition to the required GATA Registration and the submission of the Internal Control Questionnaire (ICQ). If you have not done so already, please register at http://www.illinois.gov/sites/GATA/Grantee/Pages/default.aspx and follow the registration process. Please have your Data Universal Number System (DUNS) number; SAM CAGE code; verify you are not on the Federal Excluded Parties List; and confirm you are in good standing with the Illinois Secretary of State.

After you register, you will receive an e-mail with a link to the Internal Control Questionnaire. Please note completion of Registration and submission of ICQ are due 30 days after the award date.
