WAG 02-09-05-c: Provider Staffings and the Mediation Process

relocated textStaffings with the provider are conducted monthly for cases in Cook FCRCs. Downstate, staffings are conducted with the provider as needed.

Before the staffing:

  1. (Provider) At least 3 workdays before the meeting date, sends the FCRC Employment and Training Participant List (Form 4333).
    1. Includes the name and status of each client the provider handles.
    2. Indicates on Form 4333 if an in-depth discussion is needed.
  2. (Provider) For in-depth discussions, notifies other interested parties at least 3 workdays before the meeting. Other interested parties may be providers of 3rd party treatment or other professionals involved in the client's plan.
    1. (Provider) Completes Section A of Employment and Training Staffing (Form 4334) for each client who needs an in-depth discussion and sends it to the FCRC with Form 4333.
    2. (FCRC) Complete Section D of Form 4334.
  3. (FCRC)  Resolve any discrepancies between the list and FCRC information before the monthly meeting.

At the staffing:

  1. (FCRC/Provider) 1. Brings the case file and any other background information related to each client to the meeting.
    1. (Provider) Brings copies of Forms 2151A and any revised RSPs to the meeting.
  2. (Provider) For each client, reports the number of hours in Pay-After-Performance and Supplemental Activities for Work First clients, level of cooperation in job search, other assigned activities, and changes to the RSP.
  3. (FCRC) After the regular staffing, updates the RSP in WVS, if needed.

In-Depth Staffings 

  1. (FCRC/Provider) Review progress sheets for each client identified on Form 4333 as needing an in-depth review.
  2. (FCRC/Provider) Review all resources available to the client.
  3. (FCRC/Provider) Discuss appropriateness of current activities.
  4. (FCRC/Provider) Discuss new strategies to improve client participation.
  5. (FCRC/Provider) Share information which may be unknown to one another.
  6. (FCRC/Provider) Reach a decision on the appropriate next steps for the client.
  7. (FCRC/Provider) Review and finalize Form 4334.
    1. Indicate any changes recommended for the client's RSP (e.g., change of work assignment, change in work schedule, placement in another activity).
    2. (FCRC) Schedule and hold a meeting with the client to follow-up on the changes to the RSP.

      NOTE: The follow-up meeting may include other parties. 

  8. (FCRC) Keep a copy of Form 4334.
