5.0 INTRODUCTION

The Cornerstone scheduling component has been designed with the flexibility for each clinic to design the clinic schedule(s) according to individual clinic needs.

Family Case Management:

Part of the case manager's responsibility to their clients is to refer and/or schedule services established in the care plan. Referrals are made when external providers offer the services needed. These referrals are tracked by the Cornerstone system to see that the participant receives the services.

Scheduling is the process that maps the participant's service needs to the available time slots for that service, to assure that the service is delivered to the participant.

The basic referral process consists of matching the participant's service needs with the list of available providers. After determining the resource provider, the case manager may contact that provider and schedule an appointment on behalf of the participant.

The scheduling component deals only with services provided within the clinic. The case manager examines the participantservice needs and the participant's current schedule to make the proper appointments for the participant.

WIC:

The referral component of the Cornerstone system allows WIC staff to record and track referrals made to other health services on behalf of WIC participants. The scheduling component is used for the scheduling of certifications, follow-up visits, nutrition education sessions, and food instrument (FI) pick-up.

Immunization Program:

For immunizations, the scheduling component indicates when a participant needs additional vaccines. The scheduled date will be for a future immunization clinic that is available nearest to the actual date for the participant's protection.

5.1 GROUP APPOINTMENT (SC01)

Overview

  • Group Appointment (SC01) is used to inquiry on all of the appointments scheduled for those participants within a group or family. If a participant is not in a group, no appointments will be displayed.
  • The information displayed is only found at the local clinic.
  • This screen is used for inquiry purposes; however, it is possible to delete a group or familyappointments on this screen.

Details

WAN:

If the group has been active in other Cornerstone sites, the user can view that information by pressing the F2 shared data key to access the WAN. (Please refer to Chapter 2 System Environment under 2.8.2 Accessing the WAN for more information.)

The F2 shared data key is pressed, the delete option is no longer available on this screen if data is retrieved from the WAN.

Fast Path Keys:

The F9 fast path key will take the user to the Schedule Appointment (SC02) screen.

Step by Step Instructions

  1. Verify that the information in the Participant Standard Processing Block (PSPB) belongs to the participant currently being processed. Any member of a group can be selected to get all group information. (Please refer to Chapter 2 System Environment under 2.6.1.1 Participant Standard Processing Block (PSPB)/Participant Browse for more information about the PSPB.)
  2. Press Ctrl+F1 to activate the Group Appointment window, if necessary.
  3. The screen will display the most recent group appointment record entered for the participant. Use the Pg Up and Pg Dn to scroll through previous participant group appointment records.
    • To delete a group appointment record, press F7. The Delete the Current Record pop-up window will be displayed. Select Yes to delete the record, or No to cancel.

5.2 SCHEDULE APPOINTMENT (SC02)

Overview

  • The Schedule Appointment (SC02) is used by all programs to record and maintain clinic appointments for participants.
  • This screen displays all the appointments for the requested schedule and date.
  • Appointments can be added, edited, and deleted. Time slots can also be blocked out.

Details

Selecting an Appointment:

The Schedule Appointment (SC02) allows the user to select a schedule, identify resources, and find the next available based on information entered through the use of several pop-up windows.

The Schedule Appointments window will display time slots down the left side of the screen and resources (Employees, Exam Rooms, Types of Service(s), etc.) across the top of the screen. Time slots can be blocked out on the Schedule Closed Dates (AD19) screen to indicate times when the clinic is closed or will not be able to offer a specific service. The resource columns are established on the Clinic Schedules (AD04) screen to reflect the local agencyresources and schedule.

An appointment cannot be scheduled for a participant with a "Death Date" entered on the PA03 (Participant Enrollment).

WIC:

When a participant is scheduled for a WI " (Individual Education) or WK_ (Follow-up) service on this screen, the "Education Scheduled" number on the WIC Assessment (AS02) screen will increase by 1. In addition, the scheduled education contacts are compiled on the WIC Education Summary Report (HSPR0108). (Please refer to Chapter 11 Reports under 11.4 WIC Education Summary Report (HSPR0108) for more information.)

A warning message will be shown when a WIC Certification visit is being scheduled for a child participant who will be over the age of five (5) at the time of the scheduled appointment.

Fast Path Keys:

The available fast path key is F9 to the Clinic Schedules (AD04) screen.

Step by Step Instructions

Manually Selecting an Appointment:

  1. In the "Schedule ID" field, type in the correct code or use F1 Help. (Please refer to Chapter 2 System Environment under 2.5.2 Keyboard Functions for more information about F1 help.)
  2. Press Enter to go to the "Date" field and type in the date to be scheduled or press F1 to display the calendar. Use Pg Up and Pg Dn to move from month to month. Use the arrow keys to highlight the appropriate day of the week. Press Enter and the date selected will fill in.
    • The date must be the current date or a future date up to one year.
  3. Press Enter to display the Schedule Appointments window. Press Ctrl+F1 to activate the Schedule Appointment window, if necessary.
    • If the clinic is not open on the date selected, a message This clinic is not open on this day. Choose another day or schedule will be displayed.
  4. The screen will display any scheduled appointment times. Use the arrow keys and/or Pg Up and Pg Dn to highlight the appropriate time slot.
    • To add an appointment for a participant, press F5. The Appointment Information pop-up window will be displayed.
    • To edit an appointment for a participant, press F6. The Appointment Information pop-up window will be displayed.
    • To delete an appointment for a participant, press F7. The message Confirm Delete? will be displayed. Select Yes to delete the record, or No to cancel. If deleted, the message "Appointment deleted" will be displayed in the top right corner of the screen.
    • To block out an appointment time, press F5. The Appointment Information pop-up window will be displayed. In the Service field, enter BLOC - Block Out Appointment Time.
  5. In the "Service" field, type in the correct code or use F1 Help.
  6. Press Enter to go to the Part ID field and type in the correct ID number.
    • If a participant is carried forward from a previous screen, the ID number and name will be displayed. Verify that the information belongs to the participant currently being processed.
    • If the ID is not known, the user may go to any screen with a Participant Standard Processing Block (PSPB) and select the participant. (Please refer to Chapter 2 System Environment under 2.6.1.1 Participant Standard Processing Block (PSPB)/Participant Browse for more information about the PSPB.)
    • If the current participant is part of a group, the user can press Ctrl+G to view the members of the participantgroup (Group Browse). Use the up and down arrow keys to highlight a new participant, then press Enter to select the participant. The new participantID number and name will be displayed.
  7. The Date and Time fields are not editable and will default with the date and time slot selected.
  8. Press Enter to go to the Length field. This field defaults to the length of time that was entered in the Increments field on the Clinic Schedules (AD04) screen.
    • The appointment length can be changed, but must be a multiple of 15 minutes.
  9. The Resource / Slot# field is not editable and will default to the resource or slot selected.
  10. Press Enter to go to the Appt With field and type in the appropriate Cornerstone employee ID number or use F1 Help.
  11. Press Enter to go to the Comment field and enter any comments pertaining to the appointment.
  12. The Appt Made By field will auto-fill with the ID and name of the employee making the appointment.
  13. The On field will auto-fill with the date the appointment was made.
  14. Press F4 to save the appointment. A message Record added or Record edited will be displayed in the top right corner of the screen. The participant's name will be displayed in the schedule.

Next Available Appointment:

  1. From the Schedule Appointment screen, press F10. The Next Available Appointment pop-up window will be displayed.
  2. To select the schedule(s), use the up and down arrow keys to highlight the appropriate schedule. Press Enter to move the schedule from the Schedules window on the left to the Schedules to Search window on the right.
    • When selecting two or more schedules to be searched, the schedules selected must all use the same time slot increments.
  3. Press Tab to move to the Date From field and enter the date to begin searching for the next available appointment.
  4. Press Tab to move to the Date To field and enter the end date to stop searching for the next available appointment.
  5. Press Tab to move to the Time Needed for Appointment section. The time will default to the minimum time slot increment. Use the left and right arrow keys to highlight the arrow buttons.
    • To increase the amount of time needed for the appointment, highlight the "Up Arrow" button and press Enter to increase to the desired time.
    • To decrease the amount of time needed for the appointment, highlight the "Down Arrow" button and press Enter to decrease to the desired time.
  6. Press Tab to move to the Resource Number (Optional) field and type in the assigned Resource Number to search for the staff member's next available appointment. This step is optional.
    • The Resource Number (Optional) field is only available if one appointment is selected in the Schedules to Search window.
  7. Press Tab to move to the Browse button. Press Enter to view the next available appointments based on the entered criteria. Skip to Step 11.
    • To cancel the search, press Tab to move to the Cancel button. Press Enter to cancel the search and return to the Schedule Appointment (SC02) screen.
  8. The Next Available Appointment Browse window will open, and a list of all the available appointments will be displayed. Use the arrow keys or the Pg Up and Pg Dn keys to scroll through the list of next available appointments. Press Enter to select the appointment.
    • Selecting an appointment will display the schedule and the day selected on the Next Available Appointment window. The user will need to select the correct time slot.
  9. The Schedule Appointments window will be displayed on the appropriate day. Use the arrow keys and/or Pg Up and Pg Dn to highlight the appropriate appointment time.
  10. Press F5 to add the appointment. The Appointment Information pop-up window will display.
  11. In the "Service" field, type in the correct code or use F1 Help.
  12. Press Enter to go to the Part ID field and type in the correct ID number.
    • If a participant is carried forward from a previous screen, the ID number and name will be displayed. Verify that the information belongs to the participant currently being processed.

Field Definitions

Field Name Length Field Type Definition
Schedule ID 3 Code An identification number assigned by the Cornerstone system to a particular schedule.
Date 8 Date The schedule ID date.
Service 4 Code A code that indicates the type of service the participant is scheduled to receive.
Part. ID 14 Alpha/Numeric A unique system-generated identification number which is assigned to the participant at enrollment.
Name 30 Assigned The name of the participant for the Part. ID shown.
Date 8 Date The date of the appointment.
Time 5 Choice User chooses with the highlight bar the start time of the appointment.  (Note: The column on the far left has been frozen and everything else scrolls.)
Length 4 Numeric Length of the appointment.  Must be a multiple of 30 minute increments.
Resource/Slot # 2 Assigned The number of resources available or slotâ€TMs available of a class in the selected schedule.
Appt. With 9 Alpha/Numeric The unique identification number assigned to clinic employees who the appointment was made with; used for accessing the Cornerstone system.
Comment 45 Alpha/Numeric Space to enter comments regarding the appointment.
Appt. Made By 9 Alpha/Numeric The unique identification number assigned to clinic employees who made the appointment; used for accessing the Cornerstone system.
On 8 Date The date the appointment was made by the clinic employee.
Date From 8 Numeric The date to begin searching for the next available appointment.
Date To 8 Numeric The date to stop searching for the next available appointment.
Time Needed for Appointment 2 Numeric The amount of time needed for this appointment.
Resource Number 2 Numeric The Resource Number for a specific resource on a schedule.

5.3 CLASS SCHEDULE (SC03)

Overview

  • The Class Schedule (SC03) is used to schedule participants for classes.
  • All participants scheduled for a class on a specific date are displayed in a list.
  • Class schedules can be added, edited, and deleted.

Details

A schedule ID and date are required in order for the class to be viewed and for a participant to be added to the class.

The Class Schedule (SC03) screen identifies the closed times for that schedule or for the clinic as established on the Schedule Closed Dates (AD19) screen.

Scheduling a participant for an appointment is no longer allowed if the participant has a Death Date entered on the PA03 (Participant Enrollment).

The user cannot schedule a class when the Master schedule is closed.

WIC:

The Class Schedule (SC03) is primarily used in scheduling participants for required group education sessions in the WIC program, although it is available for all programs. When a WIC participant is scheduled for a WE " (Group Education) service on this screen, the "Education Scheduled" number on the WIC Assessment (AS02) screen will increase by 1. Additionally, the scheduled education contacts are compiled on the WIC Education Summary Report (HSPR0108). (Please refer to Chapter 11 Reports under 11.4 WIC Education Summary Report (HSPR0108) for more information.)

Step by Step Instructions

  1. In the Schedule ID field, type in the correct code or use F1 Help.(Please refer to Chapter 2 System Environment under 2.5.2 Keyboard Functions for more information about F1 Help.)
  2. The Service field is not editable and will fill in automatically.
  3. Press Enter to go to the "Date" field. To display a calendar, type in the date of the class or press F1. Use Pg Up and Pg Dn to move from month to month. Use the up and down arrow keys to highlight the appropriate day of the week. Press Enter and the date selected will fill in. The scheduled time for this class will fill in.
    • The date must be the current date or up to one year into the future.
  4. The "Maximum" field is not editable and displays the total number of slots available for this class.
  5. The "Time" field is not editable displays the time of the class.
  6. The "Available" field is not editable and displays the total number of slots still open for this class.
  7. Press Ctrl+F1 to activate the Class Enrollment window, if necessary.
    • To add a participant to this class schedule, press F5. The Add Participant to Class pop-up window will be displayed. Go to step 8.
    • To delete a participant from this class schedule, press F7. The Delete the Current Record? pop-up window will be displayed. Select Yes to delete the record, or No to cancel.
  8. In the Participant ID field, type in the correct participant ID.
  9. Press Enter to go to the "Last Name" field and type in the participant's last name.
  10. Press Enter to go to the "First Name" field and type in the participant's first name.
  11. Press Enter to move the cursor past the First Name (i.e., the last) field to activate the participant browse. The "Participant Browse - Local" pop-up window will be displayed in the middle of the screen. Use the up and down arrow keys and/or Pg Up and Pg Dn to highlight the appropriate participant and press Enter. (Please refer to Chapter 2 System Environment under 2.6.1.1 Participant Standard Processing Block (PSPB)/Participant Browse for more information about the PSPB.)
  12. If the current participant is part of a group, the user can press Ctrl+G to view the members of the participantgroup (Group Browse). Use the up and down arrow keys to highlight a new participant, then press Enter to select the participant. The new participantID number and name will be displayed.
    • Press Esc to exit out of the program pop-up window without selecting a new participant.
  13. Press Enter to go to the "Comments" field and type in any relevant comments about the participant in the class.
  14. Press F4 to save the record. A message "Record added" will be displayed in the top right corner of the screen and the participant will be listed in the Class Enrollment window.

5.4 PARTICIPANT APPOINTMENTS (SC04)

Overview

  • Participant Appointments (SC04) are used to inquire on scheduled services a participant.
  • This screen can be used to assist the clinic staff when scheduling or reviewing appointments made for a participant.
  • The information displayed is only found at the local clinic.
  • This screen is for inquiry purposes; however, it is possible to delete appointments on this screen.

Details

If a participant is terminated in one category and certified in another category, the clinic will need to delete any previously scheduled appointments.

WIC:

If the group has been active in other Cornerstone sites, the user can view that information by pressing the F2 shared data key to access the WAN. (Please refer to Chapter 2 System Environment under 2.8.2 Accessing the WAN for more information.)

The F2 shared data key is pressed, the delete option is no longer available on this screen if data is retrieved from the WAN.

  • Clinic Appointments (SC05) are used to inquire on scheduled clinic appointments.
  • This screen is for inquiry only.

This screen will list the time, participant, service, and resources for each appointment.

Step by Step Instructions

  1. In the Appointment Date field, type in the date of the appointment or press F1 to display a calendar.
    • Use Pg Up and Pg Dn to scroll through the months and use the arrow keys highlight the date.
    • Press Enter to select the date.
  2. Press Enter to display the appointments for the "Appointment Date."
  3. Press Ctrl+F1 to activate the Clinic Appointments window, if necessary. Use the arrow keys and/or Pg Up and Pg Dn to scroll through and highlight the appointments.

5.6 STAFF APPOINTMENTS (SC06)

Overview

  • Staff Appointments (SC06) are used to inquire on appointments scheduled staff appointments.
  • This screen is used to delete appointments for individual clinic staff.

Details

This screen will list the time, participant, service, and resources for each appointment.

Step by Step Instructions

  1. Verify that the information in the Employee Standard Processing Block (ESPB) belongs to the employee currently being processed. (Please refer to Chapter 2 "System Environment" under "2.6.2 Employee Standard Processing Block (ESPB)/Employee Browse" for more information about the ESPB.)
    • Use F1 Help when the cursor is in the first Employee ID field to display the available employee names. (Please refer to Chapter 2 System Environment under 2.5.2 Keyboard Functions for more information about F1 Help.)
  2. Press Ctrl+F1 to activate the staff appointment window, if necessary.
  3. In the "Schedule Date" field, type in the date of the appointment or press F1 to display a calendar.
    • Use Pg Up and Pg Dn to scroll through the months and use the arrow keys highlight the date.
    • Press Enter to select the date.
  4. Press Enter to display the appointments for the "Schedule Date."
  5. The screen will display the most recent group appointment record entered for the participant. Press Ctrl+F1 to activate the Clinic Appointments window, if necessary. Use the arrow keys or Pg Up and Pg Dn to scroll through the staff appointment records.
    • To delete a group appointment record, press F7. The Confirm delete? pop-up window will be displayed. Select Yes to delete the record, or No to cancel.

5.7 SERVICE PROVIDER SELECTION (RF01)

Overview

  • The Service Provider Selection (RF01) is used to record/establish a referral for a participant to an outside provider for a specific type of service.
  • Once a referral has been generated appointment date/time, and comments can be changed.
  • When this screen is saved, the selected number of copies of the referral will be printed.

Details

The available fast path keys are F9 to the Participant Referral History (RF03) screen and F10 to the Planned Services/Care Plan (CM03) screen.

Step by Step Instructions

  1. Verify that the information in the Participant Standard Processing Block (PSPB) belongs to the participant currently being processed. (Please refer to Chapter 2 System Environment under 2.6.1.1 Participant Standard Processing Block (PSPB)/Participant Browse for more information about PSPB.)
  2. Press Ctrl+F1 to activate the service provider selection window, if necessary.
  3. The screen will display the most recent service provider selection entered for the participant. Use the Pg Up and Pg Dn to scroll through previous service provider selections.
    • To add a service provider selection, press F5. Go to step 4.
    • To edit a service provider selection, press F6. Go to step 7.
    • To delete a service provider selection, press F7. The message Confirm Delete? will be displayed. Select Yes to delete the record, or No to cancel.
  4. Press Enter to go to the Type of Service field and type in the correct code or use F1 Help. (Please refer to Chapter 2 System Environment under 2.5.2 Keyboard Functions for more information about F1 Help.)
  5. Press Enter to go to the "Referred By ID field and type in the appropriate Cornerstone employee ID number or use F1 Help.
    • Pressing Enter to move past the Referred By ID field, will auto-fill the field with the current user's ID.
  6. Press Enter to go to the Referred To ID field and type in the provider ID or press F1 to go to the Provider Look-up (SV03) screen. (Please refer to Chapter 4 Service/Activity Screens under 4.3 Provider Look-up (SV03) for more information.)
  7. Press Enter to go to the Appointment Date field and type the date of the participantappointment.
  8. Press Enter to go to the Appointment Time field and type the time of the participant's appointment, using AM for "A.M." and PM for "P.M." (i.e. 09:00 AM).
  9. The cursor will move to the Hard Copies field. The default value for this field is 0. The number may be changed to 1, 2, or 3; depending on how many copies are needed.
  10. Press Enter to go to the Reason for Referral section. The "Reason for Referral" section should only be used for providing instructions or information to the participant.
  11. Press F4 to save the screen. A message "Record added" OR Record edited will be displayed in the top right corner of the screen.
    • As soon as the record is saved, the Cornerstone system will automatically print the selected number of 'Hard Copies" of the referral form.

Phone/Fax 10 Assigned The providerâ€TMs phone number and fax number.
Appointment Date 8 Date The appointment date.
Appointment Time 4/1 Numeric/Choice The appointment time with A.M. for AM or P.M. for PM.
Hard Copies 1 Numeric The number of hard copies of the referral that the printer will print out. The user can enter a number between 0 and 3.  The field defaults to “3.â€
Reason for Referral 300 Alpha/Numeric Free form text field used to record special instructions for the participant being referred.

5.8 PROVIDER REFERRAL HISTORY (RF02)

Overview

  • The Provider Referral History (RF02) is used to browse providers for a history of referrals made to them. The history is displayed for a given month and year.
  • A browse window will be built listing the participantlast name, participantfirst name, participantID, the type of service referred, and the date of the referral for the specified month and year. If it is necessary to review a different month and year, the referral date can be changed to reflect the desired date.
  • A total number of referrals made during the period will be calculated and displayed on the screen. Detailed information may be viewed for any of the referrals by placing the cursor on the desired referral and pressing F1. The Participant Referral Detail window will be displayed.

Step by Step Instructions

  1. Verify that the information in the Participant Standard Processing Block (PSPB) belongs to the participant currently being processed. (Please refer to Chapter 2 System Environment under 2.6.1.1 Participant Standard Processing Block (PSPB)/Participant Browse for more information about PSPB.)
  2. The Referral Date field displays the current month. To review a different month, type the month and year over the date displayed.
  3. Press Ctrl+F1 to activate the provider referral history window, if necessary. Use the up and down arrow keys or Pg Up and Pg Dn to scroll through provider referral history.
    • To view more referral details, press F1. A Participant Referral Detail window will be displayed.

  • The Participant Referral History (RF03) is used to browse the referral history of a specific participant.
  • The history is displayed in reverse chronological order by the date of referral (i.e. the most recent referrals are displayed last).
  • This screen is for inquiry only.
