  1. Team 1:  Support Coordination Enhancement Recommendations

    The Support Coordination Enhancement Team will define the role of service coordinators under the new case management system.

  2. Team 2: Intake and DD Eligibility 

    The Intake and DD Eligibility Team will develop strategies to help individuals and families better navigate our complex system.

  3. Team 3:  PUNS and HCBS Enrollment

    The PUNS and HCBS Enrollment Team will develop an action plan to respond to feedback received from the PUNS listening tours.

  4. Team 4:  Profile and Provider Selection

    The Profile and Provider Selection Team will continue efforts to develop a person-centered profile that describes individual abilities, preferences, goals and support needs. This team will also develop guidelines for assessment and development of the profile and determine training needed prior to requirement for service coordinators to develop the profile.

  5. Team 5:  Monitoring and Quality

    The Monitoring and Quality Team will provide recommendations for establishing better collaboration and creating synergy opportunities between PAS Agencies and service providers that lead to better quality and improved health and safety outcomes.
