Provider information concerning the DHS Provider Year-End Consolidated Financial Report (CFR):


IDHS requests that you do NOT send hard-copy financial reporting items via USPS or fax to the Office of Contract Administration. IDHS does not maintain hard-copy files of financial reporting items due to the requirement(s) that items either be loaded to the CRV by Grantee/Providers (for IDHS specific reporting items) or to the GATA Grantee Portal for GATA required items. If you have questions about where a reporting item should be loaded, please refer to the IDHS Reporting Checklist you were sent May. If you need assistance gaining access to a site, you may write DHS.OCA.FinancialReporting@illinois.gov. If they cannot assist you, they will direct you to someone who can.
