Your Transition Service Plan will focus on your personal vision, preferences, strengths and needs in home, community and work environments. Your Transition Service Plan support you with relationships, productive work, participation in community life, and personal decision-making.
Any help you need in putting together your own personal Transition Service Plan will be provided by your Pre-Admission Screening Agency.
Please remember that your PAS Agency is the first contact for you to make for services. Your ISC Agency will be the first person you will call to find out about services, your Transition Plan, and can help you with anything else you need.
