Reject Codes in the 6 Series

These codes mean that we were unable to authorize the program being billed because the individual is still authorized in a different Waiver and cannot be in both the Developmental Disabilities Waiver and another Waiver.

• Work with the Independent Service Coordination/Pre-Admission Screening (ISC/PAS) agency that did the pre-admission screening and the previous provider to submit a Service Termination Approval Request (STAR) form to close the previous services. See Section XIII. Appendices (Forms and Instructions) in the DDD Waiver Manual.
• Make sure the termination date on the STAR form coordinates with the date the new service started.
