Statewide Survey About Family-Centered Services

The Illinois Department of Human Services/Division of Alcoholism and Substance Abuse and the Women's Committee of the Illinois Advisory Council on Alcohol and Other Drug Abuse are requesting your participation in a statewide survey about family-centered services. This information will be used for two primary purposes: 1) To obtain baseline information about family-centered services in the state, particularly information that we currently do not track (i.e., the use of evidence-based practices, workforce status, agency demographics, etc.), and 2) to create a directory for treatment providers and providers of adjunct services (mental health, prevention, etc.) to make referrals.

The Family-Centered Services Subcommittee of the Women's Committee is seeking more information on how treatment providers across the state define and deliver family-centered services. Results of this survey will help drive policy recommendations, identify gaps in our treatment system, and ultimately help create an educational platform to start diminishing those gaps.

We are very excited to have our providers share whatever information they may have regarding this service. This will be an opportunity for any personnel within your agency who works in this area to contribute and have input to provide their knowledge and expertise to this endeavor. Completion of the survey requires 30-45 minutes of time.
