Summary:
In an effort to provide better customer service and to reduce work for staff in Regions 1 and 2 , a pilot to allow active cash, SNAP, and medical cases to report an address change by accessing the IDHS Web site begins 03/05/2012.
The Change of Address, Income or Assets page contains instructions for completing The Change of Address for Cash, Medical, and SNAP Customers Form, Form 2467W, and a link to the online form. There is also a link to the Voter Registration page if the customer would like to register to vote.
When the customer selects the link, The Change of Address for Cash, Medical, and SNAP Customers Form will appear in a separate window and requires entry of all mandatory fields denoted by an asterisk.
When all mandatory fields are completed and the customer selects the Submit button, an electronic version of the Form 2467W is emailed to a designated email address. The new email address is a designated address that is only accessed by specific casework staff in Region 1.
Region 1 designated staff will process the Form 2467W and change the address for all active cases in Regions 1 & 2 unless the address change is;
for a pending application
for a canceled case,
a long term care case,
in local office 180, 189, 190, 200 or 250, or
to a P.O. Box.
Form 2467W received for cases meeting the above exceptions will be emailed to the LOA of the FCRC responsible for making the change.
Region 1 designated staff will document all actions taken on Form 514 in ACM.
Region 1 designated staff will cancel cases in Regions 1 and 2 when an out of state address is reported.
Form 2467W received for Regions 3, 4, or 5 will be emailed immediately to the Local Office Administrator (LOA) of the Family Community Resource Center (FCRC) responsible for making the change.
Region 1 designated staff move the email in the Mailbox Cabinet folder labeled Processed 2467Ws so it will be automatically be saved and archived.
Staff in Regions 3, 4, and 5 should print a copy of the 2467W to include in the case record once the address change is processed.
