IDHS/SUPR is providing DOPP Guidelines for Implementation, and the Enrollment package, Standing Order forms, and Reversal Form to help potential enrollees operate and enroll in DOPP. To enroll in DOPP, an organization must complete the Enrollment Package and submit a copy of either the statewide standing order or their internal standing order.
