Action Memo: Change of Address Processed by Central Office

Summary:

  • Effective July 15, 2011, Healthcare and Family Services (HFS) Health Benefits and All Kids Hotline will assist the Department of Human Services (DHS) Help Line in processing reported change of address and phone number.
  • Customers will continue to report changes of address to the DHS Family Community Resource Center (FCRC).
  • DHS has two customer service lines.
  • The DHS Help Line and the DHS Change Report Line will continue to process reported changes.
  • Some cases are excluded from the central address change process.
  • DHS Help Line or the HFS Health Benefits and All Kids Hotline will notify the FCRC when an out of state change of address is reported or when a customer requests to register to vote.
