Policy Memo : Integrated Care Program (ICP)

Summary:

  • Effective May 1, 2011, Healthcare and Family Services (HFS) is implementing a new healthcare program called the Integrated Care Program (ICP).
  • The ICP serves seniors and persons age 19 and older with disabilities who receive medical benefits under the Aid to the Aged, Blind and Disabled (AABD) program, but are not eligible for Medicare.
  • Enrollment in the ICP is limited to persons living in Cook (excluding the City of Chicago), DuPage, Kane, Kankakee, Lake, and Will Counties.
  • Enrollment is mandatory.
  • Two health plans, Aetna Better Health and IlliniCare Health Plan, will manage the healthcare of persons enrolled in ICP. 
  • Persons previously eligible for Your Healthcare Plus, a disease management program, will begin receiving these services through the new ICP or Illinois Health Connect.
