Information Memo: Region 2 Consolidation of Long Term Care Cases

Summary:

In an effort to improve efficiency and expertise in the handling of Long Term Care cases, Region 2 is consolidating Long Term Care (LTC) and Supportive Living Facility (SLF) cases into two locations;
- the Lake County FCRC, and
- the DuPage County FCRC
Phase 1 of the consolidation is effective 04/12/11 and includes:
- LTC/SLF cases in Boone and Dekalb FCRCs will be transferred to the Lake FCRC; and
- LTC/SLF cases in Kane/Aurora and Kendall FCRCs will be transferred to the DuPage FCRC.
CN 11.15/S(pdf) was sent to LTC/SLF cases in Boone and DeKalb and CN 11.16/S(pdf) was sent to cases in Kane/Aurora and Kendall the week of April 3, 2011 to notify them of the change.
Effective 04/04/11 as part of the No Wrong Door policy changes that were implemented in 01/11, the Your Office Selection screen is added to the Online Application for Cash, Medical, and SNAP benefits.
