Filing Complaints About DD Services

People receiving services from Division of Developmental Disabilities (DDD), guardians, family members, or other representatives, may file complaints about their services using one or more of the options described below.

Medicaid Waiver Services

Services through programs such as CILA, HBS, Child Group Home, and day programs.

  1. File a Report of Abuse, Neglect or Exploitation

    • For children under the age of 18 or for anyone residing in a Child Group Home or Child Care Institution, call the Department of Children and Family Services (DCFS) at 1 (800) 252-2873 or TTY: 1 (800) 358-5117.     • For adults age 18 and over, who reside in their own home or family's home, call the Adult Protective Services (APS) Hotline at 1 (866) 800-1409. You can view the APS website through this link: Adult Protective Services website.For reporting incidents that occur at a Developmental Training site, contact the IDHS OIG at 1 (800) 368-1463.     • For individuals residing in a Community Living Facility (CLF), contact the Illinois Department of Public Health (IDPH) at 1 (800) 252-4343 or email DPH.CCR@illinois.gov. You can view the IDPH website at this link: IDPH File a Healthcare Complaint.

  2. Contact your Independent Service Coordination (ISC) Agency

    • If you do not have the ISC contact information available, you can find the agency closest to you by using the DHS Office Locator. Under "Type," select "Developmental Disabilities." Under "County," select your county.

  3. Contact the Division

    • Submit a formal complaint through the DDD Complaint Form and a DDD staff person will respond within 72 hours: DHS DD Services Complaints.
    • Contact a representative in the Division of Developmental Disabilities by calling 1 (888) 337-5267 or by sending an email to DHS.DDDComplaint@illinois.gov.

  4. Center for Independent Living Arrangements (CILA)

    • For individuals residing in a Community Integrated Living Arrangement (CILA) contact the Illinois Department of Human Services, Office of Inspector General (IDHS OIG) at 1 (800) 368-1463. To report suspected abuse or neglect, you must call. Please do not send allegations by e-mail. Doing so may slow the investigation and jeopardize confidentiality.   5. Community Living Facility (CLF)

Intermediate Care Facility for Persons with Developmental Disabilities (ICF/DD)

  1. File a Report of Abuse, Neglect or Exploitation

  2. Contact the Division

    • Submit a formal complaint through the DDD Complaint Form and a DDD staff person will respond within 72 hours: DHS DD Services Complaints form.
    • Contact a representative in the Division of Developmental Disabilities by calling 1 (888) 337-5267 or by sending an email to DHS.DDDComplaint@illinois.gov.

State-Operated Developmental Centers (SODCs)

  1. File a Report of Abuse, Neglect or Exploitation

    • Call the Department of Public Health: 1 (800) 252-4343 or email DPH.CCR@illinois.gov. You can view the IDPH website at this link: IDPH File a Healthcare Complaint.
    • Call the Illinois Department of Human Services, Office of Inspector General (IDHS OIG): 1 (800) 368-1463. To report suspected abuse or neglect, you must call.
