Action Memo: Issue Guide to DHS Services (DHS 4474) to SNAP Customers

Summary:

  • In preparation for an upcoming policy change, Expansion of SNAP Categorical Eligibility, we have revised Guide to DHS Services (DHS 4474) to yellow card stock to make it user friendly for customers and staff.
  • DHS 4474 is a TANF funded information and referral brochure that provides a list of services offered through DHS, other agencies and community partners. 
  • FCRC staff must give the yellow card stock version of DHS 4474 to all new SNAP applicants beginning 03/01/10.
  • For new applicants, staff must document that the brochure was given to the customer. AIS CAF Screen 8c and CAF Screen 38, Forms and Pamphlets Issued, are revised so that you may enter a Y response to indicate that DHS 4474 was given to the customer. 
  • Effective 03/01/10, in AIS, a Y response to DHS 4474 on CAF Screen 8c within Option E must be entered before the system will allow you to calculate expedited benefits.
  • Starting 03/15/10, in AIS, a Y response to DHS 4474 on Screen 38 must be entered before the system will allow you to calculate benefits.
  • The final CAF serves as the documentation that DHS 4474 was issued.
  • For new applications taken in Service Coordination, staff must record that DHS 4474 was given to the customer on Form 514.
  • DHS 4474 will be inserted with March medical cards for Group 22 cases, all cash cases with active SNAP benefits and will be mailed to active Category 08 cases. 
  • Starting March 2010, DHS 4474 will be mailed with SNAP Households Receiving Notice of Expiring Certification (Form 1893 and Form 4893) so that staff do not need to pass out at REDE.
  • An initial distribution of the brochure has been sent to the FCRCs.
