Early Intervention Specialist - Developmental Disabilities

Qualifications: Professional with Development Therapy credentials or equivalent experience and training who has had their credentials approved by the DHS Division of Developmental Disabilities to provide early intervention services under the Illinois Children's Support and Children's Residential Medicaid Waivers.

How do I Become A Provider?

1. You must send a copy of our Early Intervention Developmental Therapist Certificate to DHS.DDDMedProv@illinois.gov.
2. Obtain a type-1 National Provider Identifier from the National Plan and Provider Enumeration System. 
3. Submit an IMPACT Provider Enrollment application  as a Rendering/Servicing Provider .
4. Email DHS.DDDMedProv@illinois.gov to notify DHS-DDD that your IMPACT application has been submitted for State Review.
