Becoming A New Developmental Disability Provider or Established Provider Adding Services

The Department of Human Services, Division of Developmental Disabilities (DDD), is responsible for enrolling new providers and assisting established providers in adding new services to be funded by DDD. Only individuals determined to have a developmental disability are eligible for services within this service system.  Completion of the provider enrollment forms for any of the provider types does not guarantee you will be approved for funding, or that you will receive referrals for services. Before beginning the application process, see the DDD Program Manual.

IMPACT

IMPACT is a multi-agency effort to replace Illinois' 30-year-old Medicaid Management Information System (MMIS) with a web-based system that meets federal requirements, is more convenient for providers and increases efficiency by automating and expediting state agency processes.
