MR #09.18: Change to Manually Entered Illinois Link Card Transactions and Miscellaneous Changes

Summary:

  • Effective 12/17/08, the limit of three transactions with a manually entered Link card Personal Account Number (PAN) per month has been eliminated.
  • Due to recent clarification from FNS, when a customer requests replacement benefits for destroyed food using Form 1989, Request for Replacement of Destroyed Food, the Family Community Resource Center (FCRC) must issue replacement benefits within 2 days.
  • The Form 3658, Illinois Link Card/PIN Issuance Form is revised.  FCRC staff do not have to get a signed Form 3658 when mailing the Illinois Link card from the office.
  • The Application Confirmation (Form 400) and the Expedited/Emergency Food Stamp Screening Form (Form 401) are not required for Web applications.
