Action Memo: Rapid Reintegration Pilot Program

Summary:

  • The Illinois Department of Human Services (DHS), Healthcare and Family Services (HFS) and the Department on Aging (DoA) are piloting a program entitled "Rapid Reintegration" that allows qualified persons to keep more of their monthly income.  The person must be admitted for a short term stay of six months or less in a participating nursing facility and have the need to maintain a home in the community.
  • The pilot program is limited to Boone, Morgan, Sangamon, and Winnebago Family Community Resource Centers (FCRC).
  • The Community Home Maintenance Allowance (CHMA) will be increased up to 100% of the Federal Poverty Level.
  • The person must have been discharged from the Rockford Memorial or Swedish American Hospital in Rockford or Memorial Medical Center in Springfield.
  • A new form, HFS 3902 (pdf), Community Home Maintenance Allowance Physician Statement, has been created to certify short term stays in nursing facilities.
  • Item 25, Code C has been created to identify and track cases eligible for the pilot program.
  • The pilot program will run for one year, beginning December 1, 2008.
  • HFS 2378H and 2536 have been labeled for use in the pilot program.
