Appropriate Professional Credentials Not On File
This means that the professional is billing for a service for which they do not have the appropriate credentials.
- Review Section IX, subsection (A) of the DDD Waiver Manual to see what the provider qualifications are for each service.
- FAX documentation (copy of license or approval letter from the Division of Developmental Disabilities (DDD) Bureau of Quality Management (BQM)) to Provider Enrollment at 1-217-558-2799 if you believe you meet the provider requirements and there is an error on the database. Staff will correct the database.
