Program Authorized At A Different Provider
(This is also Reject Codes 6 plus a letter: Individual is Authorized For a Conflicting Program)
These codes mean that we were unable to authorize the program being billed because the individual is still authorized for CILA at a different provider or for some other fee-for-service program.
- Work with the Independent Service Coordination/Pre-Admission Screening (ISC/PAS) agency that did the pre-admission screening and the previous provider to submit a IL462-2028 Service Termination Approval Request (STAR) (pdf) form to close the previous services. See the DDD Waiver Manual (XIII. Appendices (Forms and Instructions)).
- Make sure the termination date on the STAR form coordinates with the date the new service started.
