Name Mismatch With Department of Healthcare and Family Services (HFS) Medicaid
This means that the individual's name in ROCS is not the same as the name that appears on the individual's Medicaid card (form HFS 469) and on the Medicaid Management Information System (MMIS).
- Compare the name as it appears in ROCS with the name on the individual's current Medicaid card, check it using the MEDI Web Site, or call the Toll-Free Number. Identify the difference and determine which is correct.
- Correct the name in ROCS and transmit a case information file, or
- Contact the DHS local office (Family Community Resource Center) and request in writing that they correct the name.
