We are pleased to inform you that you have been selected to apply for the Adult Home-Based Support Services Program (HBS) for persons with developmental disabilities through the State of Illinois. The selection came from the Prioritization of Urgency of Need for Services (PUNS) database, which is now being used as the Division's official waiting list. You are now invited to contact your Individual Service Coordination (ISC) agency to submit an application to the State for services.

If not already completed, the ISC agency will work with you to conduct an eligibility determination. You must be found eligible for the HBS Program before an application can be submitted. If you are not found eligible, you will not be able to apply. In the event you are found ineligible, you will be informed about your appeal rights.

Through the Adult HBS Program, you can receive a variety of services, such as respite care, personal care, habilitation, therapies, day programs, and behavioral services. Your ISC agency can explain the options to you.

This letter inviting you to apply for services is valid for six months from the date of receipt of this letter. We hope this will give you and your ISC agency sufficient time to consider your options and submit an application. Please keep in mind the State will not pay for services until an award letter is issued.

In order to assist you, we have attached a checklist that will help you as you complete the steps in this process. If you are unsure of the contact information for your ISC agency, please call 1-888-DDPLANS during business hours and, when prompted by the recording, type in your ZIP Code. You will be connected with your local ISC office. The people there can find answers to your questions and help you through this process.
