Applying for State Employment
Create a master copy of your application:
- Complete the CMS-100 application leaving box #1 and written signature blank.
- Make several copies.
- Fill in box #1 and sign the copy for each position you would like to test.
Application is located in the FORMS Section of the Website.
For written tests: thoroughly complete the CMS-100 application; when ready to test, bring the application with you to one of the following test locations:
