• To comply with state law, the maximum rates for funeral and burial claims increase effective 07/01/07.
• The increase applies to a claim on funeral and burial services where the date of death is on or after 07/01/07.
• Funeral and Burial Reimbursement Claim (Form IL444-094) is being revised to show this change.
