Summary:

• To comply with state law, the maximum rates for funeral and burial claims is increasing by 3.5%.
• The increase applies to claims on funeral and burial services where the date of death is on or after 01/01/06.
• Funeral and Burial Reimbursement Claim (Form IL444-094) is being revised to show this change.
• A historical table of funeral and burial rates is added to WAG Chapter 25.
