WAG 26-04-03: HFS 222, Substitute Parental Care/Supplemental Child Care Statement of Services Render

HFS 222 is used to approve direct payment to Substitute Parental Care/Supplemental Child Care employees who are Qualified Providers (see PM 06-16-03)

NOTE: HFS 222 is used only by Special Projects Unit (SPU) staff.

(SPU) Instructs clients and employees:

• to return HFS 222 to SPU after the end of the approved period or the end of each month, and
• that HFS 222 must not be submitted to SPU before the end of the request period or the end of the month.
