WAG 20-26-03: Referrals

PM 20-26-03.

1. (FCRC) Use HFS 2151, Inter-Agency Referral, to refer client to treatment program.
2. (FCRC) Distribute HFS 2151:
   • original to treatment agency,
   • copy to client, and
   • copy in case record.
3. (FCRC) Document verbal referral if HFS 2151 is not used.
