LTC facilities provide services based on the client's medical and/or social condition.
LTC facilities may provide various levels of care. The whole facility may be approved by HFS for a care level, or a distinct part of the facility may be approved for a specific care level. Private facilities may provide sheltered
care without additional certification.
The level of care a person needs is based on the physician certification. The facility is responsible for notifying the Family Community Resource Center about a change in the client's level of care. The Family Community Resource Center must verify that the facility is licensed to provide
care at the new level and make sure the client's level of care is changed.
