PM 20-05-01: Notice of Restriction

WAG 20-05-01.

A client receives a written notice telling them they are being restricted. HFS chooses a primary care physician, pharmacy, or HMO. The provider will be reasonably close to the client's home. HFS's choice will be for the entire period unless the client chooses to change the provider (see PM 20-05-03-b).

The notice also tells the client that:

• they've used more medical services than needed for their medical condition;
• HFS will choose a primary care physician or pharmacy;
• there is a toll-free number they can call to get questions answered; and
• they have the right to appeal the restriction.
