PM 20-04-02: Provider Requirements

WAG 20-04-02.

To be part of the medical program, a provider must:

• have a license from the State, if their profession is licensed by the State. If the profession is not licensed, the provider must meet HFS standards; and
• have a written agreement with HFS.

Providers may also need to meet other requirements based upon the type of service provided.
