"Mental health facility" means any licensed private hospital or hospital affiliate, institution, or facility, or part thereof, and any facility, or part thereof, operated by the State or a political subdivision thereof which provides treatment of persons with mental illness and includes all hospitals, institutions, clinics, evaluation facilities, mental health centers, colleges, universities, long-term care facilities, and nursing homes, or parts thereof, which provide treatment of persons with mental illness whether or not the primary purpose is to provide treatment of persons with mental illness. Facilities with Inpatient Mental Health Treatment Programs and Residential Facilities include:
- Hospitals with Inpatient Mental Health Treatment Programs
- Nursing Homes with Inpatient Mental Health Treatment Programs
- Supervised Transitional Residential Programs
- Specialized Mental Health Rehabilitation Facility
Facilities with Inpatient Mental Health Treatment Programs are required to report the following:
- Non-Adjudicated Admissions.
- Adjudicated as a person with a mental disability.
- Persons that are determined to be a "Clear and Present Danger." *
- Persons that are determined to have a "Developmental Disability."
- Persons that are determined to have an "Intellectual Disability."
Supervised Transitional Residential programs are required to report the following:
- Admissions of individuals to the supervised transitional residential program.
- Persons that are determined to be a "Clear and Present Danger." *

Non-Adjudicated Admissions and Adjudicated Admissions are required to be reported to the FOID Mental Health Reporting website, within 7 days after Admission. (Mental Health and Developmental Disabilities Confidentiality Act 740 ILCS 110/12(b))
Clear and Present Danger events are required to be reported to the FOID Mental Health Reporting website within 24 hours. Developmentally Disability events are required to be reported within 7 days of the determination. (Mental Health and Developmental Disabilities Confidentiality Act 740 ILCS 110/12(b))
