GA-Senior Farmers Market Nutrition Program (26-444-80-0170)

Grant Application Information

  1. Eligibility for FY26 Bureau of Family Nutrition (BFN) exception/renewal grants are limited to current providers who are prequalified and in good standing.
  2. Each applicant must continue to be GATA Pre-Qualified and registered in the CSA.
  3. Applicant organizations must complete the FY26 ICQ before an award can be made.
  4. The ICQ is available in the Illinois GATA Grantee Portal and should be completed as soon as possible.
  5. Applicant organizations must submit a complete FY26 Uniform Application for Grant Assistance (pdf) and a Grantee Conflict of Interest Disclosure(pdf) to DHS.OFWContracting@illinois.gov  before an award can be made. Please include CSFA# on the email subject line. All fields on pages 2 and 3 are required. For line #45 for "Amount Requested from the State," please enter the SFY26 award amount provided by the program administrator.
  6. The Bureau of Family Nutrition reserves the right to request additional information from the applicant to assist in FY26 funding and programmatic decisions.
  7. Any changes made to grantee mailing/billing address or Administrator/Executive/CEO name/email should be reported ASAP to DHS to ensure proper information is reflected in the contract documents.

Application

All applicants for Senior Farmers Market Nutrition Program Program funding, must submit the following document electronically by June 30, 2025.

Once all applications have been received, DHS will determine a contract amount and this will be communicated electronically to the e-mail address that submitted the grant application.

Submission

The completed grant application document and conflict of interest form is to be emailed by June 30, 2025 to the email address DHS.OFWContracting@illinois.gov  with the subject line stating:

  • Last 4 digits of CSFA #0170
  • Provider Organization Name

For example: 0170 Jane's DV Agency or 0170 HOPE Agency

Note: Everything is being tracked by the CSFA number, so it is important that all required materials and correspondence contain this information.
