This program assists customers with various disabilities in obtaining and maintaining competitive integrated employment for at least 90 days. Providers are expected to employ a variety of techniques and services necessary in obtaining and maintaining community based competitive employment consistent with the employment outcome goal identified on the Individualized Plan for Employment (IPE) with a matching IPP. Services and training may include but are not limited to assessments and evaluations of job readiness, specialized training, career development services, trial work, transferable skills, social and personal characteristics, vocational skills, computer skills, communication skills, interviewing skills, employment preferences, and job seeking skills. Job retention services, including observation and monitoring as well as direct on-the-job supports including job coaching are also included and designed as follow- up services to ensure job stability and job performance.
Staffing's between the customer, Provider, and IDHS-DRS will occur every eight weeks from the time of the referral and continue throughout the life of the case.
The payment structure is based on the following Milestones: Rates as of 7/1/2024
Total value of all Milestones is $9,582. Rates per Milestone are listed below with the descriptions.
Assessment and an Individualized Provider Plan (IPP)-$1,100 - These must follow and contain the components listed in the CRP program manual. This service can be billed one time per customer. The Assessment and IPP are required to be submitted to the IDHS-DRS counselor for review and approval to bill for this service.
15-day Milestone-$2,545: Paid after 15 days, with competitive, integrated stable employment. Verification of employment will be submitted with all Milestone billing. This will include wage records, or paycheck information. Employment/placement information, customer progress and stability information must be submitted to bill.
45-day Milestone-$2,545: Paid after 45 days, with competitive, integrated stable employment. Verification of employment will be submitted with all Milestone billing. This will include wage records, or paycheck information. Customer progress and stability information must be submitted to bill.
90-day Milestone-$3,392: Paid after 90 days, with competitive, integrated stable employment. Verification of employment will be submitted with all Milestone billing. This will include wage records, or paycheck information. Customer progress and stability information must be submitted to bill.
