GA - High Risk Family Case Management-Cohort #1(26-444-80-3383)

1. This renewal application is for HRFCM Cohort #1 providers who were awarded SFY25 HRFCM funding for specificed communities who wish to continue providing services in those same communities in SFY26.
2. Eligibility for FY26 BMCH renewal grants is limited to current providers who are prequalified and in good standing.
3. Each applicant must continue to be GATA Pre-Qualified and registered in the CSA.
4. Applicant organizations must complete the FY26 ICQ before an award can be made. The ICQ is available in the Illinois GATA Grantee Portal and should be completed as soon as possible.
5. Applicant organizations must submit a complete FY26 Uniform Application for Grant Assistance (pdf) and a Grantee Conflict of Interest Disclosure to DHS.OFWContracting@illinois.gov  before an award can be made. Please include CSFA# on the email subject line. All fields on pages 2 and 3 are required. For line #45 for "Amount Requested from the State," please enter FY25 award amount as a placeholder.
6. The Bureau of Maternal and Child Health reserves the right to request additional information from the applicant to assist in FY26 funding and programmatic decisions.
7. Any changes made to grantee mailing/billing address or Administrator/Executive/CEO name/email should be reported ASAP to DHS to ensure proper information is reflected in the contract documents.
