GA - Prevent Child Abuse Support (26-444-84-2905)

To be eligible for a contract, all grantees must complete the Pre-Qualification Process, Internal Controls Questionnaire, and Grant Application. The Grant Application is to be completed for every program/grant.

FY26 Application:  Eligibility is limited to current Prevent Child Abuse Support grantees funded under opportunity number 25-444-84-2905. Current grantees will receive a continuation application packet via email.

At the time of application, all Bureau grantees must submit for each grant program the following documents:

• Grant Application (pdf)
• Program Plan - From the continuation application email.
• CSA Budgeting/Tracking System and Registration Information - Refer to the instructions in the continuation application email.
• Grantee Conflict of Interest Disclosure

The completed narrative documents must be emailed by April 24, 2025, to DHS.HomeVisiting@illinois.gov with the subject line including:

• 2905 (the last 4 digits of CSFA)
• Provider Organization Name
• Contact Person's Name

For example: 2905 ABC Community Center - Jane Smith

Note:  Everything is being tracked by the CSFA number, so it is important that all required materials and correspondence contain this information.
