Reintegration Funding for Individuals Transitioning from State Operated Developmental Centers (SODC) to Home and Community Based (HCBS) Waiver Services

Purpose:

Division of Developmental Disabilities (DDD) has created Reintegration Funding that offers $50,000 in additional funding to 24-hour, Host Family and Intermittent CILA or Community Living Facility providers to facilitate a successful and stable transition of an individual from an SODC into Adults with Developmental Disabilities waiver services. Providers qualify for $10,000 no more than 30 days prior to an individual moving into their home, $10,000 after an individual has lived for 6 months in their home, $10,000 after an individual has lived for 1 year in their home and a final payment of $20,000 after the individual has lived in the home for 2 years with the goal to assure the successful and stable transition and reintegration of the individual into the community through waiver services.

Scope:

  • Reintegration Funding offers targeted funding for providers for additional supports not already being funded through the rate or other funding mechanisms such as staffing, supports and facility improvements identified as needed to support individuals as they settle into their new home and their new life in the community.
  • Providers should work with the individual, the Independent Service Coordination (ISC) agency and the Bureau of Transition Services (BTS) to identify any necessary and additional activities, services, supports, or facility needs that would help ensure a successful transition and acclimation to the new home and community.
  • Reintegration Funding is separate and distinct from the Money Follows the Person Program and will not duplicate funding and services that are offered through that program.
  • Individuals can access Reintegration Funding of $50,000 one time. If an individual returns to an SODC and then returns back to the provider, the provider can continue to invoice for the remaining reintegration funding based on the length of total time the person resides in the home. If an individual moves to a different home in the middle of the two years, the new home/provider will be able to bill for the remaining reintegration funding.

Process for Payment:

  • The provider can submit a reintegration funding request and invoice for the initial payment of $10,000 no more than 30 days prior to an individual moving into their home, a request for $10,000 after the individual has lived in the home for more than 6 months, $10,000 once the individual has lived in the home for 1 year and a final invoice for $20,000 after an individual has lived in the home for more than 2 years. They will submit these requests to BTS (melissa.reynolds@illinois.gov) and include documentation showing that the individual transitions from an SODC and has lived in the home for the appropriate amount of time, providing the name and social security number of the individual who moved in, and the date they moved into the home.   • BTS will review and approve the request and send it to the Bureau of Reimbursement and Program Support (DHS.DDDBCR@illinois.gov).
  • Reimbursement and Program Support will process a C13 for each payment and the funding will be sent to the provider. All submitted documents will be kept with the C-13 paperwork for historical and review purposes.
  • Provider claims must be submitted within the State Fiscal Year or lapse period for the SFY for which the placement threshold occurs.
  • Annually, DDD will review the list of providers who received a reintegration payment to ensure the process is working and is appropriate.

Reintegration Funding Request/Invoice Letter should include:

  • Provider invoice number
  • Organizational Name
  • Organizational FEIN
  • Contact Person
  • Contact Phone
  • Contact Email
  • Home address for the Reintegration Payment
  • BALC ID number
  • Date of first individual moves in
  • Name and social security number of that person
  • Requested amount
  • Amount previously received if the request is for payments 2-4
