  1. Program Description
    Firearm violence has deeply harmed neighborhoods, communities, and the entire State of Illinois, both through the immediate loss of life and the long-term, harmful effects of trauma experienced by victims, witnesses, and community members. As one of its multiple funding efforts to reduce firearm violence in the most heavily impacted communities across Illinois, the IDHS-Office of Firearm Violence Prevention (OFVP) intends to fund non-profit organizations to establish and strengthen community based collaborative groups of RPSA grantees and community stakeholders to mobilize community resources and develop community specific Safety Action Plans. These activities will include oversight of RPSA victim assistance funding and will include the development of the Community Safety Action Plan with Community revitalization recommendations. The Community Conveners will make consensus recommendations to the Office of Firearm Violence Prevention and local law enforcement on how to reduce violent conflict. The Community Safety Action Plans will inform the Office of Firearm Violence's annual report to the Illinois General Assembly and will advise strategy for allocation of resources for violence prevention and community restoration efforts.
  2. FY26 Continuation Application
    Current Providers will receive a continuation application packet via email. Providers may request additional copies of the continuation application by sending an email to DHS.ViolencePreventionServices@illinois.gov
    1. This continuation application is for Year 2 under CSFA #444-80-3484. It is a 12-month project period which spans two state fiscal years. The twelve-month project period (grant term) will run from 4/1/26 - 5/31/27.
      1. Eligibility is limited to current RPSA Community Convener grantees funded under CSFA# 25-444-80-3484 and is limited to current grantees.
      2. Each applicant must be registered in the Illinois GATA Grantee Portal and must continue to be GATA Pre-Qualified.
      3. Applicant organizations must complete the FY26 ICQ before an award can be made. (Accessed through the Grantee Portal.) If the FY26 ICQ is not available in the Grantee Portal prior to the application due date, grantees should submit their application without the ICQ and will be notified when it becomes available.
      4. Applicant organizations must submit a complete FY26 Application Packet to DHS.ViolencePreventionServices@illinois.gov by due date listed in the continuation application. Include "CSFA# 444-80-3484" on the subject line.

      2. Application Summary Page and Program Narrative UPDATE
      3. FY26 Budget must be entered into CSA by application due date
      4. FY27 Budget must be entered into CSA by application due date
    3. The Bureau of Violence Prevention reserves the right to request additional information from the applicant to assist in FY26-FY27 funding and programmatic decisions.
