  1. Program Description
    The Violence Prevention Services program that utilizes evidence-based programing and services to engage individuals at the highest risk of firearm violence, including street outreach, case management, and victim services. This program model includes utilizing "credible messengers" with lived experience and community ties to mediate conflicts and disrupt cycles of violence/retaliation between informal street groups/street organizations, crews, cliques, and street networks. These programs increase positive outcomes for individuals most likely to harm someone or be harmed by firearm violence, strengthen others affected by violence, and contribute to building safer communities.
  2. FY26 Continuation Application
    Current Providers will receive a continuation application packet via email. Providers may request additional copies of the continuation application by sending an email to DHS.ViolencePreventionServices@illinois.gov
    1. This continuation application is for Year 2 under CSFA #444-80-3422. It is a 12-month project period which spans two state fiscal years. The twelve-month project period (grant term) will run from 1/1/26 - 12/31/26.
      1. Eligibility is limited to current RPSA Violence Prevention Services grantees funded under CSFA# 25-444-80-3422 and is limited to current grantees.
      2. Each applicant must be registered in the Illinois GATA Grantee Portal and must continue to be GATA Pre-Qualified.
      3. Applicant organizations must complete the FY26 ICQ before an award can be made. (Accessed through the Grantee Portal.) If the FY26 ICQ is not available in the Grantee Portal prior to the application due date, grantees should submit their application without the ICQ and will be notified when it becomes available.
      4. Applicant organizations must submit a complete FY26 Application Packet to DHS.ViolencePreventionServices@illinois.gov by due date listed in the continuation application. Include "CSFA# 444-80-3422" on the subject line.
    2. A complete application packet includes:
      1. Uniform Application for Grant Assistance
      2. Application Summary Page and Program Narrative UPDATE
      3. FY26 Budget must be entered into CSA by application due date
      4. FY27 Budget must be entered into CSA by application due date
    3. The Bureau of Violence Prevention reserves the right to request additional information from the applicant to assist in FY26 funding and programmatic decisions.
