Grant Application Information
- Eligibility for FY26 Bureau of Family Nutrition (BFN) renewal grants is limited to current providers who are prequalified and in good standing.
- Each applicant must continue to be GATA Pre-Qualified and registered in the CSA.
- Applicant organizations must complete the FY26 ICQ before an award can be made.
- Applicant organizations must submit a complete FY26 Uniform Application for Grant Assistance and a Grantee Conflict of Interest Disclosure(pdf) to DHS.OFWContracting@illinois.gov before an award can be made. Please include CSFA# on the email subject line. All fields on pages 2 and 3 are required. For line #45 for "Amount Requested from the State," please enter FY25 award amount as a placeholder.
- The Bureau of Family Nutrition reserves the right to request additional information from the applicant to assist in FY26 funding and programmatic decisions. FY26 Caseload assignments and funding amounts are being determined now.
- Any changes made to grantee mailing/billing address or Administrator/Executive/CEO name/email should be reported ASAP to DHS to ensure proper information is reflected in the contract documents.
Eligibility to Apply
To be eligible for a Healthy Local Food Systems (HLFS) contract, applicant providers must have completed the five grantee pre-award requirements:
- Authentication
- Registration
- Pre-Qualification
- Fiscal and Administrative Risk Assessment (ICQ)

Application
All applicants for Healthy Local Food Systems funding, must submit the following documents electronically:
- HLFS - Grant Application (pdf)
- HLFS Budget - completed in the Contractual Service Agreement (CSA) Tracking System - CSA Budget and Registration Information
Submission
All required HLFS application materials and correspondence must contain the CSFA number 2540.
