GA - Supplemental Nutrition Assistance Program-SNAP Education (26-444-80-0675)

Grant Application Information

  1. Eligibility for FY26 Bureau of Family Nutrition (BFN) renewal grants is limited to current providers who are prequalified and in good standing.
  2. Each applicant must continue to be GATA Pre-Qualified and registered in the CSA.
  3. Applicant organizations must complete the FY26 ICQ before an award can be made.
    • The ICQ is available in the Illinois GATA Grantee Portal and should be completed as soon as possible.
  4. Applicant organizations must submit a complete FY26 Uniform Application for Grant Assistance and a Grantee Conflict of Interest Disclosure(pdf) to DHS.OFWContracting@illinois.gov  before an award can be made. Please include CSFA# on the email subject line. All fields on pages 2 and 3 are required. For line #45 for "Amount Requested from the State," please enter FY25 award amount as a placeholder.
  5. The Bureau of Family Nutrition reserves the right to request additional information from the applicant to assist in FY26 funding and programmatic decisions. FY26 funding amounts are being determined now.
  6. Any changes made to grantee mailing/billing address or Administrator/Executive/CEO name/email should be reported ASAP to DHS to ensure proper information is reflected in the contract documents.

Application

All applicants for Supplemental Nutrition Assistance Program-SNAP Education funding, must submit the following document electronically by June 30, 2025.

Once all applications have been received, DHS will determine a contract amount and an assigned caseload for each applicant, and this will be communicated electronically to the e-mail address that submitted the grant application.

Submission

The completed grant application document is to be emailed by June 30, 2025 to the email address DHS.OFWContracting@illinois.gov  with the subject line stating:

  • Last 4 digits of CSFA #0675
  • Provider Organization Name

For example: 0675 Jane's DV Agency or 0675 HOPE Agency

Note: Everything is being tracked by the CSFA number, so it is important that all required materials and correspondence contain this information.
