Applicant Eligibility

1.Scope of Services

Project Goals and Description: The project aims to improve transition, competitive integrated employment (CIE), and independent living (IL) outcomes for students with disabilities ages 10- 24 and eventually across Illinois. To that end, the project will develop a replicable and scalable innovative service delivery model that builds a seamless, cohesive, and effective partnership among State Vocational Rehabilitation Agency (SRVA), State Education Agency (SEA), local educational agency (LEA), centers for independent living (CILs) and university partners.

This contract is related to the Illinois Pathways to Partnerships Project (IPPP), an innovative model demonstration project funded by the US Department of Education Rehabilitation Services Administration, aimed at improving transition outcomes, competitive integrated employment (CIE), and independent living outcomes for students with disabilities ages 10- 24 across the State. IPPP will deliver an innovative Pre-Employment Transition Services (Pre-ETS) model that includes services and supports for families as well as students. It will begin transition support as early as age 10 and follow students through school, aiming to increase the number of students who go on to post-secondary education and competitive integrated employment. Paxton-Buckley-Loda CUSD #10 will perform several key functions in accordance with DRS' approved grant application, including:

a. identify students for participation IPPP

b. designate staff to assist with the delivery of the curriculum of the IPPP model

c. collaborate with assigned CIL to determine intervals at which curriculum will be taught

d. hold events for students and their support systems to explain the model to students and their families

e. collaborate with partners to exchange data on participating students and evaluation data

f. participate in comprehensive training and technical assistance, as directed

g. participate in the advisory group

h. facilitate positive student outcomes by:

i. placing students in internships, apprenticeships, or age-appropriate work-based learning sites that are compatible with students' interest and needs or,

ii. providing students appropriate assistive technology or devices to facilitate post-secondary education, internship, apprenticeship, or competitive integrated employment opportunities.

For procurements conducted in BidBuy, the State may include in this contract the BidBuy Purchase Order as it contains the agreed Supplies and/or Services.

2.Deliverables

a) No later than _______, 2025, LEA will provide IDHS a list of participating Students, including, at a minimum, the following:

a. Students' contact information

b. Students' coordinated educational (IEP), 504 Plan, or Vocational (IPE) plan (whichever is applicable)

b) No later than July 1, 2025, LEA must hold 2 (two) events for participating students and their families in each fiscal year. The first event should introduce the IPPP model while highlighting Pre-ETS and related topics. The second event should be a progress report on execution of the IPPP model.

c) Beginning no later than _____, 2025, LEA must facilitate the placement of participating students in internships, apprenticeships, or age-appropriate work-based learning sites that are compatible with students' interest and needs -or- LEA must provide participating students appropriate assistive technology or devices to facilitate post-secondary education, internship, apprenticeship, or competitive integrated employment opportunities.

d) Upon execution, LEA must collaborate with CILs to develop schedules in which the curriculum will be administered to all participating students.

e) No later than ___________, 2025, LEA will enter into a data sharing agreement with the University of Illinois, Centers for Independent Living and other relevant partners for the purposes of collaboration among project partners.

f) No later than ___________, 2025, LEA will provide IDHS a list of staff designated to assist with the delivery of the curriculum of the IPPP model. For the remainder of the term of this contract LEA will provide regular updates on staff who are designated to assist with curriculum delivery, as required by IDHS.

g) No later than ___________, 2025, LEA will designate students, parents, teachers, or administrators for participation in the IPPP Advisory Group, as approved by IDHS or its designee. This individual can be a participating student, parent, teacher, or administrator.

h) No later than __________, 2025, LEA will participate any comprehensive training and technical assistance opportunities offered by the University of Illinois and Illinois Institute of Technology (IIT).

i) No later than __________, 2025, LEA will assist with the development of practice briefs, tool kits, or other project activities in support of the IPPP, if requested by IDHS or its designee.

j) Submit quarterly and annual reports in a form prescribed by the Department.

3.Performance Measures

a) Number of students identified to participate in the IPPP with coordinated educational (IEPs), or 504 Plans or vocational (IPE) plans developed--whichever is applicable.

b) Number of staff identified to be trained on the IPPP model.

c) Number of events scheduled held in support of the IPPP model.

d) Number of students/support systems/staff identified for participation in the advisory committee.

4.Performance Standards

LEA's will be expected to meet all deliverables listed in section 2 within the prescribed timelines. Performance will be based on the LEA's ability to meet all deliverables, as reported in the reports described in section 2(j). Including but not limited to:

a) Identify at least 13 students annually with copies of each coordinated educational (IEPs), or 504 Plans or vocational (IPE) plans to participate in the IPPP (100%)

b) Designate no less than 1 staff to be trained on IPPP model. (100%)

c) Hold one event to introduce the IPPP model. (100%)

d) Hold one event for a progress report on execution of the IPPP model. (100%)

e) Designate at least 1 individual to serve on the advisory committee. (100%)
