Request for Applications (RFA) for Stabilization Homes (SH)

DDD is very excited to announce that our Request for Applications (RFA) for providers to provide Stabilization Services has been posted!

As a reminder, there was legislation passed that increased the current capacity of stabilization homes. There are currently 8 short-term stabilization homes. The legislation will add an additional 8 long-term homes and 2 short-term homes. DDD encourages all current CILA providers with expertise in behavioral health supports to consider applying. The deadline for applications will be July 1.

STAT Application

Specialty Telehealth and Assessment Team (STAT) Service Provider Application

Thank you for your interest in becoming a STAT provider. Please review the qualifications and requirements before completing the application linked above. You will submit it for review and approval to IDHS-Division of Developmental Disabilities Clinical Team at DHS.ClinicalServices@illinois.gov. Providers can begin providing the service when they receive an IDHS-DDD letter of approval in addition to successfully completing all IMPACT enrollment requirements.
