The Housing MI Support program offers access to support services and mental health services to address the needs of individuals with a serious mental illness (SMI) residing in the permanent supportive housing. The individuals are offered flexible support services that are designed to build independent living and tenancy skills, which will enable them to maintain their housing. Having access to support services and other needed services will reduce the risk of returning to homelessness and for them to live as independently as possible in their communities.
The Grantee will offer access to support services and individualized mental health services to individuals who reside in the permanent supportive housing (PSH) , in accordance with current policies of the DMH Permanent Supportive Housing model. The Grantee will ensure that the individuals receiving permanent supportive housing support services have a serious mental illness (SMI) and have access to services that are delivered in accordance with HFS Title 89 Rule 140.The Grantees must exhaust all other resources, including but not limited to Medicaid, Medicare, or other insurances to ensure the Department of Human Services, Division of Mental Health is the payor of last resort.
