IDHS: DRAFT - Reintegration Funding for Individuals Transitioning from State Operated Developmental Centers (SODC) to Home and Community Based (HCBS) Waiver Services

Division of Developmental Disabilities (DDD) has created Reintegration Funding beginning January 1, 2025, that offers $50,000 in additional funding to 24-hour, Host Family and Intermittent CILA (I-CILA) or Community Living Facility (CLF) providers to facilitate a successful and stable transition of an individual from a State Operated Developmental Center (SODC) into Adults with Developmental Disabilities waiver services. Providers qualify for $10,000 after an individual has moved into their home, $10,000 after an individual has lived for six months in their home, $10,000 after an individual has lived for one year in their home and a final payment of $20,000 after the individual has lived in the home for two years with the goal to assure the successful and stable transition and reintegration of the individual into the community through waiver services.
