Primary Food Supplier

Do you primarily source your groceries from a wholesaler or a retail store?

• Provide the name of the retail store where you primarily source your WIC Foods.
• If you source from a wholesaler, provide the name of the Wholesaler along with a contact name, email, and phone number for the Wholesaler from whom you primarily source your WIC Foods.

Primary Infant Formula Supplier

Federal and State WIC Regulations require that WIC vendors purchase all infant formula provided to WIC cardholders from authorized wholesalers and retailers. The list of authorized formula suppliers in Illinois can be found at Authorized Formula Suppliers.

If your primary infant formula supplier is not listed in the dropdown, you must provide contact information for your supplier. The department will work with them to determine eligibility for being added as an authorized supplier before your application is approved.
